Health Home is a free service program that integrates and coordinates health care for Medicaid members with complex medical, behavioral, and social needs. The goal of this program is to help our Medicaid members access all the care services they need to stay healthy. Members enrolled in Health Home will be assigned a Care Manager who will work with them to create and achieve specific goals that help them become more independent and healthier.
Who can enroll in Rendr Care Health Home?
To enroll, you must:
- Be 21 years or older
- Have active non-restricted Medicaid
- Have two or more chronic conditions; or have HIV; or have a serious mental illness; or have Sickle Cell Disease
- Have a need for care coordination that if left without help, may put your health at risk
What are some of the free services provided by Rendr Care Health Home?
Once you are enrolled, you will be assigned a Care Manager. Your Care Manager can:
- Find you a doctor
- Set up and remind you of doctor’s appointments
- Coordinate transportation to medical appointments
- Coordinate your food, housing, entitlements, and other resources/needs
- Coordinate mental health and substance use services
- Coach you on wellness and exercise activities for health goals
- Follow up on medical and social needs if you are hospitalized
- Assist you in obtaining Home Care services
- Help you renew your Medicaid enrollment
